Contest Entry Form

For your Official Contest Entry, please send an E-MAIL to shortmenofstyle@forthefit.com before June 15th 2008 which includes ALL of the following information:

1. Full Legal Name and Contact Information (including mailing address, e-mail address and telephone number where you can be reached (*telephone number used ONLY to contact you in the event of a winning entry).
2. Your actual Height (in bare feet!)
3. AGE and full date of birth, M/D/Yr (must be 18 or older at time of contest entry to participate)
4. an acknowledgment that you have read and accept the official contest rules, by first reading the rules and then copying the following into your e-mail message:

“ Yes, I certify that the information provided in my official “Short
Man of Style” contest entry is true to the best of my knowledge
and belief. Also, I certify that I have read and agree to be bound by
the official contest rules available online or by sending a SASE to
P.O. Box 781167, Orlando, FL 32878. “

5. a photograph (in .jpeg format and no larger than 5MB in file size), of yourself (or your nominee), looking your short, stylish best. If you are not submitting photo of yourself, you must notify the nominee to let him know you are entering on his behalf, so that he may provide approval of use of such photo. By entering, you and/or the nominee agree to waive any rights you and/or nominee may have to the photo/essay submitted.
6. and a 150 word or less essay that describes what makes you (or your nominee) 2008’s National “Short Man of Style.”

ONLY COMPLETE ENTRIES WILL BE CONSIDERED.

No purchase necessary. Void where prohibited. The National "Short Man of Style” contest is open to legal residents of the 50 United States & D.C., 18 & older as of February 1, 2008.
